Registration Forms are for those members who want to invite
their friends, classmates or family members who don't have
time or computers or access to the internet in order to
register online. After filling up, you can either scan
and email them to me, or just register online on behalf of the
new member. |

Identification Card

This Official ID should be printed in color, in a hard-paper. The size should be
similar to any ordinary Business Cards, then type legibly your name
on the above box, then sign it on the lower-right box and paste your
colored ID on the space provided therein.
Your ID number will
be represented by your personal URL on the BSM website,
therefore, please write clearly your link on the line after the
www.batangsanmateo.com/. For verification of your personal
link, please email me at
slazh@batangsanmateo.com.
Make sure, however, that your email address
should be the same as in your registration form in our
database. |

Sticker with Logo |
I request that you
download and print these stickers in color and post them in
your cars, barangays, schools and such
other conspicuous places so that we will be
known worldwide. These are the same stickers that are now being
posted in various vehicles in San Mateo, Rizal.
You
can also design your own stickers. |
